Partner Services at Oak Hill Supportive Living
At Oak Hill our residents and their families have the peace of mind of knowing that in addition to the wonderful lifestyle that VIVA! offers we are committed to meeting all of their health and wellness needs. In coordination with our wellness partners our residents who qualify are able to receive the following services, all in the comfort of their home:
- Routine health and wellness program including weight monitoring, blood pressure checks, nurse consultation, medication management and coordination of licensed medical services
- On-site primary care physicians and ancillary including vision, dental, audiology and podiatry
- Physical, occupational and speech therapies (Medicare A&B)
- Home health services
- Hospice services
Additional services that are also available at Oak Hill are lab services and diagnostic testing, behavioral care and social services, medication delivery, a dietician to oversee the menus and provide consultation, medical supplies that can be delivered to the residents home, ability to order diabetic footwear, and fall prevention programs. Providing all these services offers our residents the ability to access healthcare services care without ever leaving their home.
