Physicians, pharmacists, home health nurses, physical and rehabilitation therapists, social workers, psychiatric nurses and hospice specialists all serve as an extension of the Pathway team. All providers are carefully selected for quality, share Pathway’s holistic VIVA! philosophy and work together to provide coordinated, seamless care. VIVA! Plus helps residents live well and recover more quickly when not well.

Easing the Transition
Let’s face it: moving to a senior living community can be a difficult transition. At Victory Centre of Bartlett, residents can use the help of a VIVA! Plus Transition Coach to help them adjust to their new living situation. The coach is a licensed social worker who works with staff and welcoming neighbors to help new residents establish a routine, get to know the staff, meet fellow residents and become involved in the community’s programs and activities.
Ticket to Wellness
Bartlett’s Ticket to Wellness program has Wellness and Community Life professionals working together to promote holistic health and well-being for each resident. With the help of the Resident Care Manager and Community Life Manager, each resident sets personal wellness goals based on the Seven Dimensions of Wellness—social, emotional, spiritual, environmental, vocational, intellectual and physical.
If, for example, a resident would like to get physically stronger, then he or she may be encouraged to participate in yoga, chair tai chi or the “Six Pack Ab Attack” core strengthening program led by a physical therapist. Residents pick which programs to attend and then set goals. After the plan is created, it is reviewed by a physician. A copy is given to the resident, so he or she can track success. The Wellness and Community Life managers monitor residents’ participation and provide ongoing motivation and encouragement. That’s the ticket to holistic health!
Colorful calendars make selecting programs easier
Victory Centre of Bartlett uses a color-coded activity calendar to communicate monthly programs and activities to residents. The colors provide an easy reference guide for residents to help them determine which programs they would like to attend based upon the wellness goals they set for themselves as part of their Ticket to Wellness. There is a different color for each of the Seven Dimensions of Wellness.
Better care means better outcomes
Not only is accessing care providers made simple, but the VIVA! Plus team results are outstanding. Working with our trusted healthcare partners, Pathway has reduced hospital 30-day readmissions by one third. Overall hospital admissions are on the decline and also the number of falls and injuries from falls.
Victory Centre of Bartlett’s unique approach to care emphasizes dignity not only in life but in death as well. The community’s unique end-of-life philosophy earned national recognition with an Assisted Living Federation of America (ALFA) “Best of the Best” award.
Helping residents through their final journey is an honor
Victory Centre’s goal is to do whatever is possible to help make every day count for their residents—up until the last. This means, when appropriate, helping residents die in place, at home, in comfort and with additional support from the VIVA! Plus hospice team and other ancillary support providers. When a resident does pass away, the Executive Director sends a call out that he or she is “leaving for the last time.” As part of Bartlett’s tradition, staff and residents line the front hallway in a final, silent tribute. The community flag, designed by residents, is then lowered to half mast for 24 hours in honor of each resident’s passing.
